The referral process for professionals


Sovereign Health accepts professional referrals from clinicians and other treatment professionals seeking options for their patients in need of treatment for substance abuse, mental health and co-occurring disorders. When our admissions team receives a referral from a medical or treatment professional, our goal is to make the process as simple and convenient as possible. Our team collaborates and communicates with referring professionals to ensure patients receive the most effective treatment.

When we receive a referral from a mental health or addiction professional, our admissions team is available to assist the professional and patient along every step of the way. Professionals who would like to refer a patient to one of our facilities may contact our 24/7 helpline to speak to a member of our admissions team or email us at info@sovhealth.com. Our admissions team will help to expedite the referral process by providing professionals and patients with more information about our facilities, treatment programs and residences.


Once a mental health or addiction professional has referred a patient to one of our facilities, he or she can contact our admissions team at any time to hear more about the admissions process or our programs. An admissions representative contacts both the professional and patient within 24 hours to complete a prescreening assessment. The assessment collects background information about the patient’s presenting condition and reasons for seeking treatment to ensure that the patient will be a good fit for one of our programs.

Once the preadmissions process is complete, the admissions representative will schedule an intake date with the patient and begin to prepare for his or her arrival at our facility. During admissions, we ask patients to sign a consent form so that we may keep the referring professional regularly updated on treatment progress and include the professional in discharge and continuing care. The treatment coordinator or another staff member confers with the professional within 72 hours of the patient’s intake if any follow-up information is required.

Discharge and continuing care

The professional’s role in the treatment process and continuing care is vital for the patient’s continued success in his or her recovery. With the patient’s permission, our treatment team communicates with the professional throughout the entire treatment process, including admissions, discharge and continuing care to ensure that the patient’s transition back to his or her normal life is as comfortable and supportive as possible.

As a treatment center and an organization fully committed to compliance with the Health Insurance Portability and Accountability Act (HIPPA), we make privacy and confidentially a top legal and ethical priority. While we encourage all of our patients to authorize release of information to treatment professionals, please be advised that Sovereign Health has a legal obligation to follow the direction of the patient and may not be able to provide the professional with all or any of the information requested.
