Recovery from mental illnesses, substance abuse and co-occurring disorders is a complicated and long-term process. As an effort to reduce relapse and help patients achieve long-term recovery, Sovereign Health provides a continuing care program to patients after they have completed their treatment. Sovereign Health’s Recovery Management Program empowers patients to manage their recovery with the help of supportive staff and resources. Our committed multidisciplinary team — consisting of clinicians, case managers and other mental health professionals — is dedicated to preventing relapse and providing patients a continuum of care to help them achieve, manage and maintain long-term recovery.

The following Sovereign Health locations provide continuing care to patients after they complete their substance abuse, co-occurring disorders or mental health treatment programs:

  • Sovereign Health of San Clemente, California
  • Sovereign Health of Los Angeles, California
  • Sovereign Health of Palm Desert, California
  • Sovereign Health of Fort Myers, Florida
  • Sovereign Health of Pompano Beach, Florida
  • Sovereign Health of El Paso, Texas

We also provide continuing care to female patients following treatment for bulimia nervosa and anorexia nervosa at Sovereign Health’s San Clemente location and to male patients over 40 years old who receive treatment through Sovereign Health of Palm Desert’s Personal Recovery Integrating Men’s Experiences (PRIME) Program.

A team approach

Technology Enhanced Aftercare and Monitoring (T.E.A.M.) is also a program offered to Sovereign Health patients after they complete their treatment. T.E.A.M. is a valuable resource that assists patients who have completed our programs in:

  • connecting with others who have completed treatment
  • learning important information about staying healthy and caring for themselves in recovery
  • gaining access to custom-designed brain games
  • receiving therapy referrals and case management, when necessary
